We also accept Medicare, Covered California, UC Berkeley Tang Center and Blue Cross SHIP.
Please bring your insurance card, Co-pay, and photo ID to all appointments.
Your insurance coverage is an agreement between you and your insurance company. Financial responsibility rests with the patient for deductibles, co-insurances and non-covered services. Insurance coverage is a variable and we cannot guarantee what services will be covered by your particular plan. If you have an HMO insurance plan, you will need to obtain a referral from your primary care provider. Please verify your insurance coverage prior to your appointment.
Co-Payments are due at time of service. For your convenience, we accept American Express, Visa, MasterCard, Discover, Care Credit, Personal Checks, and Cash.
NOTE: Minors under the age of 18 must be accompanied by an adult for their first visit. After the first visit you may send your child unaccompanied provided we have a consent form on file. The consent form found here will allow you to authorize future visits and or treatments in your absence.

Once you are approved, you can use Care Credit to help manage health, wellness, and personal care costs not covered by insurance. NO SHOW FEES
A no-show fee of $50.00 will be charged to medical dermatology patients who fail to show for their appointment and who give less than 24 hours notice of cancellation. Our no show policy is implemented as no show visits create an unused appointment slot that could have been used for a patient in need.
For aesthetics clients, your credit card will be requested to guarantee your appointment. If an aesthetics client does not show up for their appointment and does not provide notice of a cancellation 24 hours prior to their appointment, they will be charged 50% of the planned services on the card used during the reservation process.
Any last minute cancellations out of your control should be communicated to our front office staff for fee waiver consideration.
Surgical and biopsy specimens are sent to an outside lab to be analyzed. The fee for this service is a separate charge from the office visit and will be billed directly by the processing lab. We will send your specimen to a lab that is contracted with your insurance company. Pathology results are usually available 7 to 10 days after the test has been performed. Questions regarding payments for laboratory services should be discussed with the appropriate lab.
PRESCRIPTION PRIOR AUTHORIZATIONS AND REFILLS
Due to the high cost of many dermatology medications we will make every effort to obtain a prior authorization for your medication if necessary. We can also recommend specialty pharmacies, which typically have lower costs. If your insurance simply will not pay for specific medications we will gladly offer a substitute or generic alternative. Please allow two weeks for prior authorizations to be processed.
Prescription refill requests can be left on our prescription refill line Monday-Friday from 9 AM-12 PM and from 1:30 PM-5:00 PM.
